Falkirk: A recent inquiry has uncovered serious systemic failures at Polmont Young Offender Institution. These failures contributed to the preventable suicides of Katie Allan and William Lindsay in 2018.

Katie, just 21, and William, only 16, were found dead in their cells months apart. The inquiry revealed that their deaths could have been avoided with better precautions. Sheriff Simon Collins pointed out that prison and healthcare staff didn’t properly assess or share vital information about their mental health risks.

Katie had been struggling with her mental health during her time in prison. Despite showing resilience, she faced distress from being taunted by other inmates. William, on the other hand, had a history of mental health issues and was flagged as a suicide risk shortly before his death.

Sheriff Collins highlighted that almost everyone who interacted with William failed him in some way. He emphasized that the system for handling mental health referrals was broken, leading to tragic consequences.

The inquiry has led to 25 recommendations aimed at preventing future tragedies. These include improving communication among staff and ensuring that families can report concerns about a prisoner’s mental health.

Justice Secretary Angela Constance expressed deep regret over the deaths, stating that the system had let these young people down. She committed to addressing the recommendations made by Sheriff Collins to improve the situation in Scottish prisons.

This inquiry sheds light on the urgent need for reform in how young prisoners are cared for, especially regarding their mental health. It’s a wake-up call for everyone involved in the justice system to ensure that no more lives are lost due to systemic failures.
