Jacqueline McQuillan, North Lanarkshire, Scotland

Hamilton: A woman named Jacqueline McQuillan died in a hospital. Her death raised public concern, prompting an inquiry. The Crown Office has started the process for a fatal accident inquiry (FAI) to look into what happened.

A preliminary hearing is scheduled for March 7 at Hamilton Sheriff Court. The inquiry will examine the care Jacqueline received from North Lanarkshire Social Work Department from 2012 to 2018.

The goal of the FAI is to find out how she died and if any precautions could prevent similar deaths in the future. Unlike criminal cases, FAIs focus on facts, not blame.

Procurator Fiscal Andy Shanks stated that the circumstances of Jacqueline’s death warrant a discretionary inquiry. He assured her family would be updated on important developments as the court proceedings move forward.

North Lanarkshire Council has been contacted for comments regarding the situation.
